Guidance Center Record

The Personal Record is accomplished by students, as assisted by their parents or guardians, at the beginning of the school year to help the School Guidance and Student Affairs Office in providing support for the welfare of students. To ensure privacy, parents may also opt to send the accomplished form by registered mail or courier addressed to the Principal.
